How the Selection Slip works

The Selection Slip is used by those disseminating in the field to select, or choose, someone for service at Flag. Several copies of the Selection Slip are used, each with a separate purpose. When selecting someone, you must retain one copy for your records, as well as pass the Selection Slip to the person being selected.

Just fill out the forms that follow, and a copy will automatically be sent to Flag so that we can assist you to get your selectee here for services, as well as promptly pay your commissions when they start.
